REGISTRATION FOR 2011-2012 SCHOOL YEAR

REGISTRATION FOR ALL STUDENTS

Pre-kindergarten through 8th grades

- Students must be registered before entering classrooms.

- The parent signature page is required to be completed at registration. The signature page refers to the student handbook. This is a very important document and should be kept as a reference at home. Students will review the handbook the first week of school with their teachers.

- Please be sure to come with your email address so that we can log you into our new student system that will allow you to view grades and fees.

- Please be sure to bring required proof of residency with you.

- Forms available for registration on this page. Just print at home, fill out and bring to the office.

- The home language survey form is needed on file for each student.

- Free and reduced application form can be filled out any time of the year.

- Student Fees

- $30 for book rental per student.

- Please feel free to pay ahead for meals too.

OPTIONAL FORMS

- School Medication Authorization Form

- Over the Counter School Medication Authorization Form

- Please remember parents must supply the over the counter medication. Pleasant Hill will not supply any medications for students.

- Allergy Form

- Parents must complete if student has been diagnosed by a doctor with an allergy.

- Emergency Food Allergy Action Plan Form

- This form is needed if medicine needs to be given at school due to a food allergy.
